Specific support form for self-employed workers (health and social action)

The self-employed tick the box corresponding to his situation (‘Respite aid’, ‘Support for continuing employment (AMA)’, ‘Exceptional financial assistance (AFE) for invalids’) and fill in the form with the necessary supporting documents for each type of aid.

The application must be sent to the CPAM of the contractor's habitual residence.
