Assistance form for contributors in difficulty (ACED)

To apply for a CAFE, you must complete the following form and attach the following supporting documents:

· Last tax or non-tax notice for all household members

· A supporting document to shed light on the difficulties.

The form must be sent to the Urssaf via the Urssaf personal area by sending the request via the section: messaging.

After checking all the administrative criteria and supporting documents, the application is submitted to the Health and Social Action Commission of the CPSTI of the place of professional activity.

The individual contractor is informed in writing when a decision has been made by the SCTSI.
