The application for registration in the order requires the creation of an application file.

The application file must contain the application form available on the Order website.

The folder must be supplemented by the following documents :

• Presentation of the original, production or sending of a legible photocopy of a passport or a national identity card valid
• Copy of diploma of a veterinary doctor or of a diploma, certificate or evidence of formal qualifications in veterinary medicine
• For veterinarians whose diploma is not automatically recognized in France, the ministerial decree entitles them to practice in France
• One extract from criminal record less than 3 months old. This certificate shall be replaced or supplemented, in the case of veterinary surgeons who are not French nationals, by a certificate issued less than three months ago by the competent authority of the State of origin or the State from which the animal comes certifying that the conditions of good character and good repute required in that State for taking up the activities of veterinary surgeon have been fulfilled
• One handwritten declaration written in French in which, on oath, the person concerned declares that he has been acquainted with the code of veterinary ethics and undertakes to exercise his profession with conscience, honor and probity
• One proof of domicile administrative professional
• If the veterinarian wishes to carry out his profession on a shared activity, a copy of the written contract concerning that shared activity
• For the persons concerned, a copy of the contract drawn up between the veterinarian and his employer
• For the exercise as responsible veterinarian, or interim responsible veterinarian, delegated veterinarian or interim delegated veterinarian of an establishment of the same companies, a copy of the contract between the veterinarian and the company or establishment, accompanied by:
  
  • The justification that the person concerned satisfies the conditions for the exercise of his rights laid down by the rules
  • A copy of the document of the competent social body of the company appointing the person concerned and laying down his duties
• For the exercise as an assistant veterinarian in a company or establishment for the wholesale supply of veterinary medicinal products or for the clinical trial of medicinal products, any document indicating the nature, conditions and procedures for the exercise of this activity
• For the exercise of veterinary duties under the Convention in relation to a company on which an establishment manufacturing, importing or distributing medicated feedingstuffs depends, the copy of the Convention binding the veterinarian to the company.

The request is made by registered letter with notice of receipt, to the President of the Regional Council of the Order where the veterinarian proposes to establish his personal home or his administrative professional home. For businesses, the registration in the Order's table takes into account the location of their registered office.
