  1. Copy and paste the following text in word processing software:
    For the attention of the bailiff insert here his name, his mailing address and, when available, his fax number and his e-mail address:
    I/ We (*) hereby notify you/ notify you (*) of my/ our (*) agreement to participate in the simplified procedure for the recovery of the claim for a total amount claimed consisting of the following:
    • total principal amount

    • total amount of interest

    invoked by the creditor on the basis of the basis of the claim: date of the contract or statutory obligation.

  2. Complete the text with your personalized information and cross out unnecessary information (*)
  3. Print the document
